Hill City-Keystone Volunteer Application

I believe in God, the Father Almighty, Creator of heaven and earth. I believe in Jesus Christ, His only Son, our Lord. He was conceived by the power of the Holy Spirit and born of the Virgin Mary. He suffered under Pontius Pilate, was crucified, died, and was buried. He descended to the dead. On the third day He rose again. He ascended into heaven, and is seated at the right hand of the Father. He will come again to judge the living and the dead. I believe in the Holy Spirit, the holy catholic Church, the communion of saints, the forgiveness of sins, the resurrection of the body, and the life everlasting. Amen.

On behalf of myself, my heirs, successors and assigns, my participating children, and those children of which I have been appointed to act as a legally appointed representative, I waive and release all claims for damages which we may have against Love INC, and churches, religious denominations or organizations participating in a Love INC event, and any agent or employee of any such organizations, arising from our death, injury, or illness arising from our participation. I am aware of the risks associated with volunteering or participating in Love INC of the Black Hills, including but not limited to, death or injury by accident, disease, weather conditions, inadequate medical services and supplies, criminal activity and random acts of violence. I voluntarily assume all such risks. I agree that this assumption of risk is intended to be as broad and inclusive as permitted by law. Love INC can use any photos or video taken of me for use in their own publicity materials.

I believe the truths stated in the Apostles' Creed, I agree to abide by Love INC's Confidentiality Statement and have answered the above questions truthfully.

I further state that I have carefully read the foregoing assumption of risk and understand its contents, and I voluntarily sign this release as my own free agent.
