The undersigned parent/legal guardian hereby gives permission to Love INC/UNITE, for my child (named in this form above) to take part in the following activity:UNITE Fall 2025.
Should my child require immediate or emergency medical care while engaged in an activity sponsored by the Love INC/UNITE, in my absence, I hereby grant the Love INC/UNITE authority to release my child for medical treatment to such medical personnel as the Church determines appropriate under the circumstances.
In consideration for the privilege of allowing my child to participate in the above-named activity, I agree to release and hold harmless Love INC/UNITE, its officers and agents, from any liability to or responsibility for bodily injury, damage or illness to the above-identified child while participating in any youth athletic or social activity which may be directly or indirectly sponsored by the Love INC/UNITE. Further, I agree to indemnify and hold harmless Love INC/UNITE, its officers and agents with respect to any claim asserted by or on behalf of my child as a result of bodily injury, illness, or damage.

Child Photography Release Without compensation, I hereby grant permission to Love INC of the Black Hills and other participating organizations to use and re- produce photographs taken of my child. These photographs may be used for news and editorial purposes in publications, electronic reproductions (web sites), and/or brochures. In addition, I grant my permission to alter the same photos without restriction and to copyright the same. I hereby release the photographer, the journalists, and the publications or media outlets they represent, as well as Love INC and other participating organizations, from all claims and liability relating to said photographs.
